UK breast cancer screening guidelines miss 95% of women under 50 who develop disease, study finds

A landmark study by the University of Cambridge and The Institute of Cancer Research, London, reveals that current NICE GP referral guidelines miss up to 95% of women under 50 who develop breast cancer within a decade. Because 73% of these younger patients have no family history of the disease, the family-tree-focused NICE criteria referred only 1.4% of women. In contrast, the multifactorial BOADICEA algorithm identified eight times as many cases. While NICE welcomes the findings, it has declined to update its guidelines, citing resource constraints and the need for further clinical feasibility data.

NICE referral criteria failures

BOADICEA risk assessment tool

• ▪The BOADICEA model calculates risk using rare pathogenic variants, a 313-SNP polygenic risk score, questionnaire-based lifestyle and reproductive factors, mammographic density, and family history.
• ▪The BOADICEA risk model, which integrates family history, genetics, lifestyle, and reproductive factors, identified eight times as many breast cancer cases in women under 50 as the current NICE criteria.
• ▪Applying the full BOADICEA model to all women under 50 in primary care would refer 26.5% of women for further assessment, capturing 34.8% of those who develop breast cancer within 10 years.

Breast cancer under 50

• ▪Breast cancer is a leading cause of death in women under 50 in the UK, and cases in this age group have risen by approximately 1.4% annually over the past few decades.
• ▪Women under 40 in the UK who develop breast cancer are nearly 40% more likely to die from the disease than those diagnosed over the age of 40.

UK screening policy

• ▪A spokesperson for the National Institute for Health and Care Excellence (NICE) stated that current evidence does not warrant a change to its existing familial breast cancer guidelines at this time.
• ▪A companion study published in the British Journal of Cancer found strong public support among UK women aged 30 to 49 for proactive, universal invitations for breast cancer risk assessments over self-referral.
• ▪The NHS Breast Screening Programme automatically invites women for routine breast cancer scans every three years only from the ages of 50 to 71.
